Wrap OC

CSP's WRAP OC is a family-centered program and utilizes individualized interventions to build on the strengths of the family. The wraparound process is a way to improve the lives of youth and his/her family who have complex needs. The program's primary goal is for the family to achieve self-sufficiency.

Goals and outcome objectives are mutually agreed upon between the youth, family/caregivers, professionals, service providers, and both formal and informal support systems. All services are culturally and linguistically competent, family centered and strength based. Referrals must come directly from Orange County Social Services Agency, Health Care Agency or Orange County Probation.

WRAP OC is available to youth and families/caregivers 24-hours a day, seven days a week and provides the following services:
  • Individualized plan of care with safety plans
  • In-home and community based services and supports
  • Resource identification and coordination
  • 24-hour, on-call WRAP OC formal wrap team availability
  • Inter-agency service coordination
  • Youth mentoring and support
Youth and their families are guided through the wraparound process with the assistance of the WRAP OC team. The WRAP OC team consists of a Care Coordinator, Parent Partner and Youth Partner.

Care Coordinators develop, lead and coordinate the wraparound process and guild the Family Team through the successful progression of stages towards self-reliance and independence from the wrap team and wraparound process.

Parent Partners provide direct services to the family/caregivers and will be a support to the Family Team. They provide peer support, link families/caregivers with appropriate services and serve as a liaison to the family/caregivers amongst other responsibilities.

Youth Partners provide behavioral interventions, pro-social skills, problem solving and coping skills as well as consistent and positive support for the youth.
